Revision rhinoplasty

Correction after a previous nasal operation. The tissue has already been changed, so the plan is different from that of a primary procedure.

When it is considered

Revision rhinoplasty in Zagreb is considered because of a shape that does not belong to the face, a breathing problem after an earlier procedure, asymmetry, a collapsed bridge or tip, or scar tissue. It is not a second attempt at the same operation. The anatomy is different, reserve material is smaller, and the skin remembers the first incision.

It is not done while the nose is still changing shape. As a rule one waits at least 12 months from the previous operation, unless there is a reason for earlier intervention. The first operation, if it still lies ahead, is described on the primary rhinoplasty page. Primary rhinoplasty

Secondary here means the same: the nose has already been operated on. It is not a “second chance at the same operation”. Scar, lack of cartilage and impaired ventilation change what is feasible. That is why the examination is not drawn from the memory of the first nose.

If the first procedure was recent and the nose is still changing shape, waiting is not delay. Whoever touches tissue that is still settling often gets a third problem instead of a solution to the second.

Secondary and revision rhinoplasty here mean the same: the nose has already been operated on. It does not matter whether the first procedure was “only the tip” or a full correction. The tissue remembers the incision. The plan is drawn from the present nose in Zagreb, not from an operative note that no longer matches the anatomy.

What the examination must show

How much cartilage remains, what the skin is like, whether the nose breathes and what is realistic to repeat. Sometimes a smaller correction of the tip or bridge is enough. Sometimes support must be reconstructed. Sometimes the honest answer is that a further procedure will not bring what you expect.

Bring what you have: old records, a CT scan, photographs from before the first operation. The examination still rests on the present nose, not on the memory of it.

Endoscopy shows the valve and the passage that a photograph does not see. From the outside a tidy nose can breathe poorly. The reverse is also true: a crooked outer shape need not be the only reason for blockage. That is separated at the examination, not from a message.

Common reasons for coming

A remaining hump or a new irregularity of the bridge. A tip that has dropped, become pointed or asymmetrical. A nose that breathes poorly after the first operation — often the valve, not “just swelling”. Collapsed nasal bones, a wide or unstable middle vault, a visible columellar scar that bothers. Each of these findings pulls a different plan.

A “before” photograph helps to understand what the starting point was. It does not decide what may be touched now. Skin that is already thin or scarred does not tolerate aggressive thinning. That is a limit, not an excuse.

Sometimes only one detail bothers — the columella, asymmetry of the tip, a small irregularity of the bridge. Sometimes the support of the whole vault must be restored. Those plans are not the same procedure and not the same cost outline.

Grafts

If the septum no longer gives enough firm cartilage, a graft is taken from the ear (concha) or, less often, from the rib. The ear leaves a small, usually hidden scar. The rib is considered when more material or stronger support is needed. That is not a standard add-on — the decision is surgical, after the examination.

A graft from the septum, if any remains, is the first choice because it is on the same field. The ear gives thinner, more curved material — good for the tip and the alae, weaker for a long supporting strut. The rib gives volume and firmness when the rest will not stand.

A second incision is not an add-on to the package. It has its own recovery: the ear is short, the rib longer and can hurt on coughing. That is said before an appointment, not in theatre.

What revision cannot do

Revision does not return the nose to what it was before the first operation by default. The skin has already been touched, support is often weaker, and scar tissue pulls in a direction that cannot be predicted with the same certainty as in a primary procedure. If the expectation is “as if there had been no operation”, the examination must separate that from what the anatomy still carries.

A third or fourth procedure on the same nose is not the same as a first revision. Each further incision spends the reserve. Sometimes the honest answer is that it should not be touched again — or that only function is touched, not shape. That is not a refusal. It is the limit of the tissue.

A photograph from a network and “just fix this” rarely stand as a plan. One detail pulls neighbouring structures. At the examination it is drawn what may be touched, not a list of wishes.

Why one waits a year

A year of waiting is not a formality. Swelling of the tip, scar tissue and the feel of the nose change for months. Whoever operates while the nose is still settling operates on a moving target.

An earlier examination makes sense if breathing stops, if there is infection, an exposed graft or an obvious deformity that will not wait. The examination is then not a promise of a procedure — it is often a decision to wait further, with an instruction on what to do until then.

Expectations

Revision is more demanding and less predictable than primary rhinoplasty. Healing is slower, swelling at the tip lasts longer, and scar tissue can pull the shape even months after the procedure. The aim is improvement, not a guarantee of “correcting” everything that bothers you.

Recovery follows the same frame as a primary procedure, but the pace is often slower. The calendar is on the recovery page. Revision and reconstruction with a graft are usually more expensive than a primary correction because they take longer and require more material. Recovery · Fees

If the only aim is to return the nose to what it was before the first operation, that should be said at the start. Sometimes that is possible. Sometimes the skin and support no longer carry that shape. An honest answer is then worth more than another procedure.

Revision rhinoplasty in Zagreb starts with the examination, not with a message and a photograph. Endoscopy, the skin, remaining cartilage and breathing determine the plan. If the plan is not for you, the examination stops there. If it is, the next step is the cost outline for that scope and a recovery calendar that is often slower than a primary procedure.

Common questions

What if I have already had surgery and I am not satisfied?

Revision is not a repeat of the same operation. The tissue has already been changed, scar tissue changes the plan, and there is often no longer enough cartilage on the septum. That is why one waits, as a rule, at least a year from the previous procedure, unless breathing or a complication require an earlier examination. Bringing old records helps, but the decision rests on the present findings. The aim is improvement, not the erasure of everything that bothers you. If the skin, support or expectation do not allow a new procedure, you will hear that at the consultation.

Why can I not breathe after an earlier rhinoplasty?

A common reason is narrowing of the internal valve, an overly aggressive lowering of the bridge, or a septum that was not addressed, and sometimes a scar in the nasal passage. From the outside the nose may look tidy, yet air still does not pass. That is why the examination includes endoscopy, not only a comparison of photographs. Sometimes support must be restored with a graft, sometimes the septum or turbinates must be corrected. If the lining or a sinus is the main problem, a new rhinoplasty will not resolve it. The plan is drawn from the findings, after the tissue has settled.

Is a graft from the ear or rib needed in every revision?

No. A graft is taken when the septum no longer gives enough firm cartilage. The ear is a common choice for the tip and the alae. The rib when more material or stronger support is needed. That is seen at the examination, not in advance from the name of the procedure.

How much does revision rhinoplasty cost?

Revision and a rib graft are not the same cost outline. The examination is not a deposit. The ranges are on the fees page; the exact figure comes after the examination.

Can I bring records from the first operation?

Yes. Old findings, a CT scan and photographs from before the first procedure help to understand the starting point. The decision still rests on the present nose. Without an examination in Zagreb there is no plan and no cost outline.
